Sad and tragic story I hadn't heard of yet. My thoughts go out to the families of the victims and those that have to live with the consequences. I would never want to live with the thought of being responsible for the loss of lives due to my faulty work. I always crosscheck or even rethink the procedures when I get home just to be sure. There's nothing worse than not being absolutely sure.
Once again this accident shows that it always needs a chain of mistakes to lead to an actual accident. What I really wonder about is why the fuel problem hadn't been fixed as it is a primary system endangering safe flight. Usually this would be a definite reason for AOG as it's a level one finding. Did it at least seem to be fixed after the first occurence and its true cause remained hidden or was the maintenance simply not done properly? The choice of airfield is another matter. Wouldn't one want to get back down as soon and safe as possible?
